On your first visit with us, we will need to have some information from you. Please make sure you bring these things with you:
Dental Insurance Card
Radiographs no older than 3 months (we’ll take new ones if you do not have these)
Make sure to get to the office about 15 minutes early so we can take your paperwork and get you signed in.
Other Things to Note About Your First Appointment
In addition to the paperwork that we will need from you, there are a few other things to note about your initial consultation.
Avoid brushing your teeth and flossing for 24 hours before your appointment. We will do a plaque sample analysis during your visit.
Be prepared for a 1-hour appointment with Dr. Leday.
Personalized Treatment Plan
During that appointment, Dr. Leday will conduct an oral exam and discuss her plaque findings. Also, She will determine if biocompatibility testing is necessary. After the initial consultation, she will come up with a personalized treatment plan for your oral healthcare.
We are happy to accept several forms of payment including cash, check, MasterCard, Visa, and Discover cards.
For those that need financing options, we recommend Care Credit Financing. This healthcare credit card company can allow you to set up a payment plan for your out-of-pocket expenses without interest. Since we are not able to offer in-house financing or payment plans, this is your best option.
Our Appointment Confirmation Policy
We make every effort to help you remember your appointment date. Our team will get in touch with you several days in advance to confirm your upcoming appointment. If we aren’t able to reach you, we will try a second time by every means we have available.
Changes of Appointment
Our team understands that life is unpredictable and things come up. Please provide us with 3 days' notice if you need to cancel or reschedule. If you break your appointment without long enough notice, we will charge a fee. In the event of a true emergency, please get in touch with us right away so we can work with you and waive the fee.
